Prior to admission, our dedicated nursing team completes a thorough assessment to determine the appropriate level of care, ensuring a smooth transition into our community with personalized support from the very beginning. Following this assessment, we develop a person-centered support plan tailored to each resident’s unique needs, preferences, and goals, helping to promote independence, dignity, and quality of life. To ensure care remains appropriate and responsive, we also conduct reassessments on a quarterly basis, as well as whenever there is a significant change in a resident’s condition. These services include in-house physician care, pharmacy services, hospice care, rehabilitation therapies such as physical, occupational, and speech therapy, skilled nursing, podiatry, laboratory services, mobile x-ray, wound care, dental care, and psychiatric health services. Our goal is to provide convenient, coordinated care in a familiar and comfortable environment while helping residents maintain the highest possible quality of life. While we proudly partner with a variety of preferred providers, we are always happy to work with other local providers based on each resident’s personal preferences and healthcare needs.
